    Vendor rep immunization requirements

A common periop vendor set. Joint Commission and state rules still sit under the facility policy.

Vendor rep immunization requirements for hospital OR access

Before a vendor rep gets perioperative access, hospital credentialing teams usually check immunization and TB screening records against the facility’s policy. Missing titers, an expired TB screen, or an unsigned declination are common reasons a vendor file stalls. This immunization requirement checklist lists a common periop vendor set so credentialing, OR, and compliance staff can review a file in one pass. How to use this immunization checklist

With the rep’s records and your facility policy side by side, tick each item the file documents. Open items go back to the vendor before the badge is issued. Then pair it with the rep onboarding checklist, the credential expiry calculator to track TB and flu-season dates, and the training recency tracker.

    What your score means

The count shows how many common immunization items the vendor file documents — not whether the rep meets a specific facility’s policy. Accepted evidence, declinations, and TB windows vary, so check any open item with your credentialing or infection prevention team.

    FAQ

What immunizations do hospitals usually require from vendor reps?
A common perioperative vendor set covers MMR, varicella, hepatitis B, Tdap, seasonal influenza, COVID per current facility policy, and TB screening. The exact list, and which items allow a declination, is set by each facility’s policy and applicable state rules, so check it against the facility’s vendor credentialing requirements.
Does a titer count instead of a vaccination record?
Many facilities accept lab evidence of immunity (a titer) for items such as MMR, varicella, and hepatitis B in place of vaccination dates. Whether a titer is accepted, and what the lab report must show, depends on the facility’s policy — confirm with the credentialing or infection prevention team.
How recent does a vendor rep’s TB screening need to be?
The acceptable window and test types (IGRA or TST) are defined by the facility’s policy and its TB risk assessment. Reps with a history of a positive TB result are often asked for a chest x-ray or symptom screen instead of a repeat test. Use the facility’s stated window rather than a general rule.
Can a vendor rep decline a vaccine and still get OR access?
Some facilities allow a signed declination for certain vaccines, such as influenza or hepatitis B, sometimes with conditions like masking during the season. Others do not accept declinations for OR access. The facility policy decides, and the signed form should be on the vendor file.
Is this immunization checklist medical or legal advice?
No. It is an educational checklist for hospital vendor credentialing, perioperative, and compliance staff reviewing a vendor file. Requirements come from the facility’s policy, occupational health or infection prevention, and applicable state rules.

Educational only. Not a credentialing determination, not legal advice, and not a substitute for each facility’s policy. Vendor rep immunization requirements does not submit files to hospitals or GHX/Vendormate/Symplr.
